For those still looking for or at Model Releases here is a pretty simple one I came across online. It is from GuyLittlePhotography+Stock website www.guylittle.com.au.
In the Educational section there is lots of information for Students and Assistants that may be of interest. I just came across the site so cannot vouch for it.
